Client Progress Scorecard
A scorecard clients complete at milestones to measure real movement, not just how a session felt. It captures progress across the outcomes your programme promises, so wins are visible, stalls are caught early, and renewals have evidence behind them.

Answers run 0 to 10 and convert to a score out of 100 across Outcome, Application and Confidence. Run it at intervals to chart movement over time and to show a client, in numbers, how far they have come.
Real progress on the goal and confidence, but application dipped. Rebuild one daily habit this fortnight, and use this score against the last to show the client their trajectory.
| Thriving (80-100) | Big movement. Capture the win and set the next goal. |
| Strong momentum (55-79) | Good progress with one area to firm up. |
| Needs a reset (0-54) | Progress has stalled. Diagnose and adjust the plan. |

Questions about this template
How often should clients fill it in?
Monthly or at set milestones works well. The value comes from comparing scores over time.
Can clients see their own trend?
Yes. Show them their result each time so the progress becomes motivating in itself.
Can I match it to my programme?
Yes. Set the categories to the exact outcomes you promise so the scorecard measures what you sell.
